CareTrack data shows RPM program with Tanner Health driving real blood pressure gains across two states

CareTrack data shows RPM program with Tanner Health driving real blood pressure gains across two states

Remote patient monitoring is no longer just a convenience tool for healthcare providers — new data from CareTrack and Tanner Health suggest it is moving the needle on chronic condition management where it matters most: in the exam room and beyond. The program, which launched in early 2025, has expanded to every Tanner primary care location and is now producing measurable clinical outcomes that could reshape how rural health systems approach hypertension care.

Program has received a 50% increase in adoptions and has reached patients across 18 counties in Georgia and Alabama

CareTrack, a remote healthcare solutions provider, is reporting fresh clinical results from its Remote Patient Monitoring (RPM) collaboration with Tanner Health, a partnership that continues to scale across Tanner’s primary care network. The program, which began as a beta initiative, now encompasses all Tanner primary care locations and providers, with referrals and active patient numbers climbing by more than half since its inception. Several dozen more patients are currently waiting for device setup or activation, and participants span 18 counties across Georgia and Alabama, including rural communities that often lack easy access to follow-up care.

The latest data highlight meaningful improvements in blood pressure among patients with readings recorded during both their first and second 45-day stretches in the program:

  • 58.4% improved their average systolic blood pressure
  • 57.3% improved their average diastolic blood pressure
  • 47.1% improved both

“RPM gives providers a clearer picture of what is happening with a patient between office visits,” said Derek Skinner, Co-Founder and CEO of CareTrack. “As Tanner has expanded the program, we’ve been able to reach more patients, identify concerns that need follow-up and see encouraging changes in blood pressure over time. That is exactly what this type of care is intended to support.”

The data also reveal a pattern of declining clinical alerts as patients remain engaged in the program. Alert frequency dropped 18.5% during the second 45-day period and fell another 13.8% during days 91 through 135. By the third period, patient readings were 29.8% less likely to trigger a clinical alert than during the first 45 days.

Among patients who started with an average systolic blood pressure above 140 mmHg and stayed in the program through days 91 to 135, 75% improved their average systolic blood pressure. That group’s average fell from 151.6 mmHg to 143.2 mmHg — a reduction of 8.4 mmHg — and 43.8% of those patients moved from above 140 mmHg to below that threshold by the end of the period.

From April through August 2026, CareTrack identified, assessed and routed 706 clinical escalations to Tanner providers and staff under jointly established protocols. These escalations covered abnormal readings and patient-reported concerns that warranted Tanner’s attention or possible follow-up. Monthly escalation volume more than doubled during that window, rising from 103 in April to 210 in August. Meanwhile, alert frequency per 100 readings declined nearly 30% over patients’ first 135 days in the program — a sign of both program growth and improving stability among monitored patients.

Remote Patient Monitoring by CareTrack continues to deliver significant results in patient care, particularly in rural healthcare systems.

Why it matters: These findings suggest that remote monitoring can deliver sustained, measurable improvements in hypertension management — a critical concern for rural populations where follow-up care is often limited. As adoption grows and alert rates decline, the model points to a scalable path for proactive chronic care that goes beyond the clinic walls.
