Players venture into a decaying realm overtaken by plague, battling the illness and its fallout, safeguarding what remains untouched, and doing whatever is necessary to return to safety.

In Plague Keepers, participants assume the role of an Inquisitor attempting to carry out a hazardous contract inside a cordoned-off zone. They can brave the mission alone or join forces with as many as three companions to investigate dangerous locations, eliminate plague sources, retrieve valuable artifacts, examine infected sites, and hold strategic positions while keeping track of ammunition, health, and the clock. Contracts can send Inquisitors to any location and compel them to undertake any task, all while they battle grotesque plague manifestations—former humans twisted into monstrous forms, eager to pull everyone else down with them.
Once past the gate, players face a limited window to finish the mission and extract before the infected fog descends, making the surroundings far more hazardous.

Players can manipulate the surroundings to their advantage—creating new routes, restoring power generators, activating massive bells to stun foes, and more. Aggressive enemies can overwhelm through sheer numbers, pushing players to stay close, heal, endure, and fight using period-inspired guns, melee strikes, and violent close-quarters finishers.

Each contract assigns different duties and presents unique challenges, requiring players to complete tasks involving destruction, recovery, rescue, and investigation while adapting to every new mission.

The real dilemma emerges once the contract is done—extract immediately or risk staying longer for more rewards. While fighting to the extraction point is already stressful, players also have the option to linger and hunt for rarer resources as the zone becomes increasingly treacherous, introducing an entirely new level of tension.
